VA Disability Claim Checklist: What Veterans Should Gather Before Filing

A short, clear checklist can take some of the stress out of preparing a VA disability claim. The list below covers items veterans often organize before filing. Not every item applies to every claim — use it as a starting point and adapt it with a qualified professional.
Identity and service documents
- DD-214 (and any DD-215 corrections)
- Other separation documents, if any
- Personnel records relevant to your claim (orders, awards, performance reports)
Medical records
- Service treatment records
- VA medical records, if you receive VA care
- Private medical records related to the conditions you want to claim
- Test results, imaging, and specialist notes when available
Personal and lay statements
- Your own written statement describing onset, symptoms, and impact
- Buddy statements from people who served with you
- Statements from family members or coworkers who can describe impact
Symptom and impact records
- Dated symptom log (frequency, severity, triggers)
- Notes on missed work, hospital visits, or major flare-ups
- Any accommodations you've needed at work or home
Information you'll likely be asked for
- List of all conditions you want to claim
- Approximate dates symptoms began
- In-service events, exposures, or injuries that may be related
- Names of medical providers and approximate dates of treatment
